A FREE outdoor event taking place at Hughes Elementary School where the community can gather for fun, food and music provided by Johansen High School Performing Arts Association (JPAA). 

Please provide some information about your company and the product that you sell. This product can be handmade items or items related to a Direct Sales Company. JPAA and Hughes Elementary would appreciate a $25 table fee and a donation item from your company which would be raffled off. 

Once we receive your completed document, JPAA President, Lisa Honberger, will be contacting you for $25 and you can turn in your raffle item on the morning of the event. Thank you in advance for your support!
